Review: I have waited almost 6 months to post this review. When we toured the facility we were so impressed with the information shared with us. I thought it was a dream come true and thought if I can't take care of my husband anymore at home I was so grateful to be placing him here. He was there 2 months and 3 days when somehow he was allowed, in the middle of the night, without supervision and not a single bedcheck all night long to see that he had left his room and walked out AN UNLOCKED DOOR leading to the courtyard where he fell and hit his head and DIED. He was found by the day shift once he was discovered to be out of his room. I had spent many nights with him to find the night shift employees asleep on the couches in the tv room. I'm quite sure that's where they were that night.My biggest complaints other than my husband of 46 years died, the facility has NO CAMERAS, AN UNLOCKED DOOR leading to the outside, NO ALARM ON THAT DOOR, NO BED CHECKS the entire shift.I feel so bad for the day shift employees as they had such a repour with my husband and felt the loss of his death.I'm sorry my opinion of the facility change on the flip of a dime. There is legal charges pending in Tulsa Civil Court.

Review: I would give 0 stars if that was an option. I’m begging all families looking for a facility to stay as far away from this place as possible. We have fought for improvements for over a year-to no avail. Things have continued to decline to a point that it was not safe to continue to keep our beloved mother there any longer. It has NOT improved when new management took over-like promised. UNDER STAFFED to the point it is unsafe. COLD FOOD, HORRIBLE FOOD, NOT BEING ABLE TO GET IN FRONT DOORS (you have to call & wait for someone to let you in. Several times after outings with my mom with me unable to stand waiting for someone to open door, ROOMS NEVER CLEANED, SMELL IN FACILITY is horrendous. Mom sat in her own urine for 24 hours because she had fallen and wasn’t able to get up and NO ONE did a thing until we got there & called an ambulance. We were told-by the leasing agent-so many wonderful things when we moved mom in…ALL LIES. We will be taking legal action. It is heartbreaking to have your mom & others served cold food, chicken tenders so dried out that the folks there can’t eat them because of dentures, etc. No regular vitals taken, meds taken into the room and left there as if our mother was going to remember to take them herself. How many times did this occur & we don’t know? $5300 plus a month for NOTHING!!! I talked to some very high in management (not at PH) and after assuring me things would be looked into many times, they wouldn’t even call back when told they would. The onsite management is a JOKE. They sit in their offices and do NOTHING to make things better for older people who can’t use their voices-and many are afraid of poor treatment if they do complain. Luckily for us we have documentation. I have never written a review in my life, but I feel as though I have to do something to save other families from the heartbreak we and our elderly mother has faced. STAY AWAY.

Review: I have nothing positive to say about this place. They do not follow state regulations and are always understaffed. Communication from nursing staff with family is non existent and when questioned about things you are gaslit. They have let my family member suffer in isolation and caused them to miss MANY meals since they wouldn’t bring them down for meals which was in the care plan. Moved her out when management tried to deny all allegations. My cameras don’t lie. Please do not put your loved one here.
