Review: 1 star is too many. This place is horrible. My friend got transferred to this place from another facility. It was terrible. They are understaffed and the staff they employ is sub-par, slow to respond, rude, verbally abusive. The place stinks. Holes in walls the ceiling had a huge hole aprox 2x3 feet insulation hanging down. Floors are in disrepair old dirty. Management was rude evasive when questioned. Would not provide patient census or the number of staff. Don't believe any good reports. Anyone stating that this place is good is delusional. If the few good reviews are real, my guess is that they are fake reviews. Equipment is old and in disrepair. Food is slop I wouldn't give to my dogs. Speaking of my dogs, I would not feel comfortable leaving them in this facility. My friend was in six other facilities in the Tulsa area. This was the absolute worst I've seen. I could go on and on. Fortunately I was able to get my friend out of that place. Disgraceful Gracewood!

Review: My mother has dementia and our family could no longer give her the care or time that she needs. She has been at Gracewood for several months now in a memory care unit and she actually seems to be doing better. Every time we visit the facility is clean and the staff is very friendly. Mom really enjoys the food also as she has gained weight which we couldn't get her to do at home. My family highly recommends.

Review: This facility made me feel completely comfortable placing my dad in long term care. We felt guilt for not being able to care for him at home. The staff here helped me and my family come to terms with the need and went above and beyond. The facility could use some updates but the staff there is amazing! Don’t always look at the surface it goes much deeper they gave the care I knew we could not give him at home!

Review: This place is awful! I have a family member there now and I have never seen them in poor health like I've seen the other day. Food is not fit to eat! The health department needs to close this place permanently!!!

Review: Horrid conditions, understaffed, most staff seen playing on phones, no desk clerk every time I've been, no tv's in rooms, shared rooms, 1 bathroom per 2 rooms, staff heard telling obsenities, walls have holes, smells of death, no emergency button installed in room, no landscaping done outside, the building is right outside a small ghetto. I wouldnt let my dog live here let alone my mother. This isnt a place to spend your end of days, this is a place to go and die.
