Review: My dad has now been at Grace Skilled Nursing for long term care for about 2 1/2 months and he is doing so good. The nursing staff and Administrators have done a great job helping my dad settle in and be comfortable. A big shout out to Jennifer and Pat for helping me with getting my dad’s Medicaid set up and making sure everything is in place for his care. Peggy, the Activities Director text me pictures of my dad enjoying some of the activities and he always looks so happy. Diana, the barber does a great job with my dad keeping him shaved and a haircut, and she is so friendly. I appreciate all the hard work Starla puts into to her staff to make her team all work good together and keep their moral positive and the environment pleasant. My family calls my dad “Pappy” and now all the staff call him “Pappy”.After taking care of my dad for almost 41 years my husband and I decided to place him in a long term skilled nursing facility since his care has become more difficult and he has fallen several times recently. Grace was recommended by a friend and it is close to where we live which will allow us to visit every day to make sure he gets settled in well. He was admitted today and all the admin and nursing staff were very pleasant and helpful. I am hoping he gets settled in and is happy with his care at this location so he can be near us.

Review: Mom was transferred there from assisted living and they were so wonderful- Jennifer Branson- was a BLESSING to us!! I’m very happy with how they have treated us and my mother as she transitions to end of life 🙏🙏✝️✝️

Review: I hate to even give a one star but it will not let me leave a review without one. My mother went to Grace for rehab after having surgery and needing PT and OT. She was originally suppose to be there 21 days, she entered the week of Thanksgiving and was told the following week her insurance was only going to cover 13 days but then they later released her on day 12. In the 12 days she was there she only received maybe 4 PT sessions that consisted of standing once, playing bingo, talking, and a few steps with a walker. 4 days into her stay they had not once changed her bedding or cleaned her room. Told her she cant go to the restroom by herself but would never answer call lights when needed (it took over an hour and half for help when I walked down and personally asked for the nurse for tape for her bandage) and never once in 12 days offered to help with a shower or bed bath. Her meals were inconsistent on times, as a diabetic she was made to go 15 hours without a snack or meal which caused severe nausea and vomiting (meals provided were not always edible or nutritious, we had to take her food on multiple occasions). When she or the family voiced concerns their answer....we're short staffed (and this came straight from the ADON)....sorry but that is not the patients fault or problem and your lack of staff should never interfere with proper patient care. How about not fill the beds in your facility over the amount the staff can handle?? She wasn't even given mediocre care! She was released home without the ability to walk still and no help from staff to even help her into the transporting vehicle (they stood and watched her and the family struggle and never once offered to help). We have since found out from her own insurance the facility lied stating she did not need the full 21 days that 13 would be sufficient then still released her at 12. They failed to notify her Dr she was released who was unaware until we contacted them today, 4 days post release. the insurance company is launching a complaint against them on her behalf and their own. I would not send my worst enemy here. My mother entered needing rehab on one leg and now cannot walk at all due to the strain that has been put on her other leg because she was made to do everything herself. She is now immobile due to their negligence.

Review: I was admitted 4/1 discharged 5/3 bcuz of bilateral shoulder fracture. Most staff were very sufficient and PT staff exceptional. Sure,staff turnover, some cold meals, meds delayed occasionally, I wás not able to make my bed and the aids didn't w/out being asked.. 1 shower a week. Activites above average. Foodmore good than not good.
